It must be a registered institute in the CERN RadShip database https://radship.app.cern.ch/public/institute-list , with an active license at the time of shipment. Differently this test request will not be processed.

I have read and agree to the Terms and Conditions and Privacy Policy

Specifically, with my submission, I commit to:

– inform CHARM coordination of any change of test conditions with respect to what indicated in this form;

– request any change of date, or cancellation, at least 5 weeks in advance;

– perform a dry run of the setup and obtain CHARM coordination validation before any irradiation with beam;

– initiate transport formalities of the irradiated setup as soon as that will be available outside CHARM controlled area. After 5 weeks of permanence in the RP buffer zone 157/R-O21, the transport request will be forcibly activated by CHARM team, to the destination specified in the form, and the indicated Budget Code will be charged for external transport requests;

– ensure that me and all the team members taking part to the experimental activities will comply with CERN regulations for radioprotection and general safety.

I am aware that , failing to comply with all the above, my test could be revoked and access to the facility may not be granted in the future.
